We welcome New Patients.  Please remember to bring your current insurance cards and a picture ID.  Copayments are due at the time of service.  If unsure of what your copay is, please refer to your benefit booklet or contact your insurance company.

Flu season is upon us.  Please do not forget to schedule your flu vaccine.  Establish patients, please call for a Medical Assistant appointment.  New patients, please call to schedule a physician visit.  We will bill your insurance on your behalf.  If you do not have insurance or your insurance does not cover cost, our flu vaccines are $20.00.
